ABSTRACT

Objective:To explore the clinical effect of phase Ⅰ cardiac rehabilitation exercise on patients with acute myocardial infarction after percutaneous transluminal coronary intervention (PCI).Methods:Convenient sampling method was adopted, totally 84 acute myocardial infarction patients after PCI was randomized into an observation group and control group. Both groups accepted general nursing care. The observation group also accepted the phase Ⅰ cardiac rehabilitation exercise. Cardiac ultrasonic, the 6-minute walk test were used to evaluate the patients′ cardiac function and exercise tolerance, the SF-12 were used to evaluate the quality of life.Results:After repeated measurement ANOVA, before the intervention, there was no significant difference in cardiac function and quality of life between the two groups ( P>0.05); before discharge, the 6-minute walk distance of the observation group was longer than that of the control group, and the difference was significant( F value was 5.279, P=0.024). At 1 month after discharge, there were significant differences in the LVEF( F value was 8.119, P=0.006) and 6-minute walking distance( F value was 9.829, P=0.002) between the two groups ( P<0.05), analysis of the six items of SF-12 including general health( F value was 6.905, P=0.010), physical functioning( F value was 10.595, P=0.002), role physical( F value was 11.168, P=0.001), bodily pain( F value was 12.548, P=0.001), mental health( F value was 7.362, P=0.008) and vitality( F value was 13.692, P<0.001) having shown significant differences between the two groups. At 3 months after discharge, there were significant differences in the LVEF( F value was 11.156, P=0.001), 6-minute walk distance( F value was 16.554, P<0.001)and quality of life in all dimensions between the two groups ( P<0.05). Conclusion:Phase Ⅰ cardiac rehabilitation exercise can improve cardiac function and the quality of life in patients with acute myocardial infarction undergoing PCI, and enhance the exercise tolerance.

ABSTRACT

Objective:To investigate the clinical effect of esmolol on young and middle-aged patients with acute anterior myocardial infarction.Methods:Patients with acute anterior myocardial infarction from January 2008 to August 2020 were collected to obtain the basic information and clinical indicators. According to the clinical medication, the patients were divided into metoprolol group and esmolol group. The metoprolol group ( n=189) received routine esmolol, and the esmolol group ( n=104) received esmolol, intravenous injection, and then metoprolol sustained-release tablets. The clinical indexes, Gensini score, Killip grade, esmolol status and cardiac function after 7 d and 3 months of treatment were compared between the two groups. Results:Compared with the metoprolol group, the triglyceride (TG) was significantly higher, and the alanine aminotransferase (ALT) and aspartate aminotransferase (AST were significantly lower in the esmolol group (all P<0.05). The C-reactive protein (CRP), N-terminal pro-brain natriuretic peptide (NT-proBNP) and Gensini scores of culprit vessels in the esmolol group were lower than those in the metoprolol group ( P<0.05). There was no significant difference in cardiac function between the two groups within 7 d after treatment ( P>0.05). After 3 months of treatment, the left ventricular ejection fraction (LVEF) was higher and left ventricular end diastolic diameter (LVDD) was lower than those in the metoprolol group ( P<0.05). The number of postperative, ventricular tachycardia, shock and death in the esmolol group were lower than those in the metoprolol group, but the difference was not statistically significant ( P>0.05). Conclusions:Intravenous infusion of esmolol in young and middle-aged patients with acute anterior myocardial infarction can significantly improve the myocardial injury, liver function and cardiac function in prognosis.

ABSTRACT

Objective@#To explore the present perceptions of patient-empowering nurse behaviours, self-efficacy and patient activation among patients with chronic diseases, and analyze the influencing mechanism among these three variables.@*Methods@#Using convenient sampling method, a total of 485 inpatients with chronic diseases were sampled from two tertiary comprehensive hospitals in Guangzhou on the day of discharge between March and October 2017. They were surveyed with the demographic questionnaire, Patient Perceptions of Patient-Empowering Nurse Behaviours Scale , Chronic Disease Self-Efficacy Scale and Patient Activation Measure.@*Results@#Empowerment could positively predict self-efficacy (β=0.402, P<0.01) and patient activation (β=0.169, P<0.01). Self-efficacy could also positively predict patient activation (β=0.887, P<0.01). And self-efficacy, of which the effect accounted for 67.87% of the total effect, played a partial intermediary role between empowerment and patient activation.@*Conclusions@#To improve patient activation, empowerment and self-efficacy should be both emphasized and applied by the nurses, so that patients can move smoothly from a care receiver before discharge to one of self-management.

ABSTRACT

Objective To analyze the clinical effects of dexmedetomidine on postoperative agitation in children patients . Method Forty-four children undergoing surgeries in our hospital between March 2011 and March 2014 were treated with intravenous injection of 10 mL dexmedetomidine with a dose of 0.5μg/kg in 15 minutes after general anesthesia. Result The occurrence rate of postoperative restlessness was only 6.82%(3/44). Conclusion The use of dexmedetomidine combined with specific clinical nursing intervention for children undergoing surgery can effectively reduce the occurrence of postoperative restlessness , which ensures the clinical therapeutic effect and life safety.

ABSTRACT

Purpose To study the c1inica1 and patho1ogica1 features of pituicytoma. Methods Ten cases of pituicytoma were re-trieved. Their c1inicopatho1ogic and immunohistochemica1 features were studied,and the re1ated 1iterature was a1so reviewed. Results The 10 patients aged from 4 to 68 years,with 4 ma1es,and 6 fema1es. The fo11ow-up information of 9 cases was co11ected:a11 of them were survived,whi1e one fema1e recurred 2 years after operation. Histo1ogica11y,the tumor was composed of bipo1ar e1ongated spind1e ce11. Immunohistochemica11y,the tumor ce11s showed strong1y positive for S-100(10/10)and vimentin(10/10),whi1e weak or foca1 positive for GFAP(10/10)and EMA(4/10). CKpan was negative in a11 cases and Ki-67 pro1iferation index was 1ow(1% ~5%). Conclusion Pituicytoma is a 1ow-grade spind1e ce11 tumor,typica11y occurs in the se11ar region. It is most common1y found in adu1ts,especia11y in ado1escent patients. The tumor shou1d be distinguished from pi1ocytic astrocytoma and neurohypophysea1granu1ar ce11 tumor. Pituicytoma exhibits strong1y positive for S-100 and vimentin. Genera11y,this tumor behaves indo1ent1y,but some may re-cur.
